As per the Victorian Government’s re-opening roadmap, a health declaration is to be completed by staff, students, contractors and visitors each day/shift before attending University campuses or sites.
One of the most important measures to keep us all safe is to stay home if you are unwell and do not attend campus.
This health declaration is to be completed by individuals without IT Access before attend our campuses.*
Staff, Students, Honoraries and contactors who have IT access should NOT complete this form.
Instead, people with IT Access should go to https://unimelb.edu.au/coronavirus and complete the Health Declaration in ServiceNow
We remind you that by completing this declaration you are agreeing to abide by the health, hygiene and physical distancing measures required to contain the spread of COVID-19 in our community – every time you attend campus.
If you are unwell with a new illness, and are experiencing any of the following symptoms: fever, chills or sweats, cough, sore throat, shortness of breath, runny nose, loss of sense of smell or feel generally unwell, please stay home.
* Note: This does not include residents in colleges or residential accommodation, who have their own arrangements. This process does apply for staff and contractors at University-owned accommodation.
